Medical Mistakes Caused by Unclear Medical Abbreviations

The FDA in collaboration with ISMP (Institute for Safe Medication Practices) published a press release on June 15, 2006 to clarify information regarding their campaign to reduce medication mistakes caused by unclear medical abbreviations. The press release is very informative and should be reviewed by Directors of Nursing and then shared with direct care staff who are responsible for administering medications to our residents. It is also recommended that this information be shared with Medical Directors, Attending Physicians, Pharmacy Consultants and Administration. Included in this press release is the Institute for Safe Medication Practices recommended list of abbreviations, symbols and dose designations most often associated with medication errors. Also included in the press release are two websites where additional information can be obtained regarding medication abbreviations and the impact on medication errors. Please see Medication Errors (FDA) and Error-Prone Abbreviations (ISMP).
